If you’re a new patient and trying to fill out the questionnaire online before you go for your appointment,don’t it doesn’t work, even after you get the success statement online. Made an appointment,took a month to get in and then they throw six pages to fill out,very disappointed.

these guys are great if you like to see a new dentist and new hygienist every time you go in. the flip staff like crazy. Always new people every visit. oh and of course it is EXPENSIVE.
